The Comparative Study of Bile Composition from Patients with Gallstones and Common Bile Duct Stones

Abstract
In order to clarify the mechanism of stone formation in gallbladder and in common bile duct and to know more about the normal composition of bile in Koreans, along with the impact of aging on changes in bile composition, we compared the bile
compositions, such as cholesterol, total bile acid, major bile acid compositions, phospholipid, protein and calcium, from patients with gallbladder stones, common bile duct stones and control patients. The control group consisted fo patients who
has
neither gallbladder stone nor biliary disease.

@EN 1) The mean values of cholesterol and protein of bile from patients with gallbladder stone wer higher than those of patients with common bil duct stone, and those from control gallbladder(p<0.05).
2) significant differences in the mean values of bile acid, phospholipid and calcium were found in the threc groups. The mean value of these elements in gallbladder stone were lower than that in normal control, but higher than that in common
bile
duct
stone(p<0.05).
3) The mean ratio of Dihydroxy/Trihydroxy bile salt(D/T ratio) of bile from patients with gallbladder stone was higer than that from patients with common bile duct stone, and control group(p<0.05).
4) Of the bile compositions in gallbladder stone, concentration of cholesterol, including total bile acid, D/T ratio and protein, appeared to be increased with aging. But, these differences were not statistically significant(p>0.05).
5) In the amounts of bile composition with aging, all components from patients with common bile duct stone wer all lower than those from GB stone, showing no significant difference from each other(p>0.05).
6) According to age, in comparision with the bile compositions in gallbladder stone and common bile duct stone, calcium, phospholipid increased in all age, but the total bile acid, D/T ratio, protein increased in age above the 6the
decade(p<0.05).
In conclusion, the bile from gallbladder stone patients contained high concentration of cholesterol and protein, compared with the bile of normal group, but the concentration of total bile acid, phospholipid, calcium was lower than those of
normal
group.
The concentration of cholesterol, total bile acid, protein and D/T ratio in the bile of gallbladder stone patients increased with aging, but total bile acid and D/T ratio in common bile duct stone patients decreased with aging.
The author suggests that total bile acid and D/T ratio will have a different mechanism between gallbladder stone and CBD stone fromation, but discrepancy may need further study.
